Informed Consent for Testing
Prior to the beginning of the new school year and/or at the beginning of each sport season, student drivers, student athletes and parents/guardians/custodians will be required to complete and sign a “Drug Testing Consent Form,” which is included as an attachment to District Policy 227.1 Random Drug Testing.
No student may purchase a parking permit or participate in practice or competition until these forms are properly executed and on file with the appropriate Building Principal. Completed forms may be turned in to the specific team coach for athletics and to the appropriate school’s main office for paid activities and parking permits. Only one completed form is required to be submitted per student per school year, regardless of the number of sports or activities in which the student participates.
