Informed Consent for Testing
Prior to the beginning of each school year and/or at the beginning of each sport season, or when a student moves into the District and plans to join a sport or paid activity or applies for a parking permit, the student and his/her parent(s)/guardian(s) will be required to complete and sign a “Drug Testing Consent Form,” which is included as an attachment to District Policy 227.1 Random Drug Testing.
No student may purchase a parking permit or participate in practice or competition until his/her form is properly completed and submitted to the appropriate Building Principal. Completed forms may be turned in to the specific team coach for athletics and to the appropriate school’s main office for paid activities and parking permits. Only one completed form is required to be submitted per student per school year, regardless of the number of sports or activities in which the student participates.
