MASD Technology Policy Forms

Parents/guardians and/or students in Grades K-12 must acknowledge that they have read and the following technology policies:

237 Electronic Devices and Personal Technology Resources
815 Acceptable Use of Internet, Computers and Network Resources

GRADES 4-12 — For students in Grades 4-12, both the parent(s)/guardian(s) and the student must read sign the appropriate acknowledgement page(s).

GRADES K-3 — For students in Grades K-3, only the parent(s)/guardian(s) is required to sign the appropriate acknowledgement pages; however, the parent(s)/guardian(s) are asked to review the information with their student(s) prior to signing.)
